How to Access DVA Chiropractic Care at Rise Chiro
Gold Card holders can book directly — no GP visit or referral form needed. White Card holders need a signed D0904 form from their GP before their first appointment.
White Card: Get Your D0904 Signed
Download the D0904 referral form, take it to your GP, and ask them to complete and sign it before your first appointment. Gold Card holders can skip this step entirely.

Book an Initial Consultation online at your preferred location — Albion or Chapel Hill. In the comments when booking, note that you are a DVA patient — this helps our team prepare your file.
Bring Your DVA Card
Bring your DVA card on the day. White Card holders should also email their signed D0904 to admin@risechiro.com.au prior to their visit, or simply bring it along.
Gold Card Holders — No Referral Needed
If you hold a DVA Gold Card, you do not need to see your GP first and you do not need a D0904 form. Simply book an Initial Consultation, bring your Gold Card on the day, and we take care of the rest. DVA is billed directly — no paperwork, no out-of-pocket cost.

White Card Holders Need a D0904 Referral Form
If you hold a DVA White Card, the D0904 is the official DVA referral form for allied health services including chiropractic. Your GP must complete and sign it before your first appointment. Without it, DVA cannot cover the cost of your visits. Gold Card holders are exempt — no form required.
